An important message from the School Nurse:
Hello Parents/guardians,
My name is Narvelle Pledger Jr., MSN-RN, I am the full time nurse here at Calahan Street Elementary school and I have a few important notes for you all.
Allergies/Dietary restrictions:
If your student has any food allergies and/dietary restrictions, it is very important that you and your child’s physician fill out a “Special diet form” to be turned in to the School Nurse, which will be forwarded to our Food Services department. This is necessary so that we are able to accommodate your child’s dietary needs/restrictions. Without this form, we cannot guarantee that we will be able to accurately provide your child with appropriate dietary options during school days. I have attached the “Special diet form” to be filled out by parents/guardians and physician. If your child needs dietary restrictions/accommodations, please fill out this form and return to School Nurse as soon as possible. An alternative option is to have your child bring their own food to avoid any dietary issues.
Medications:
If your student requires medication during school hours, whether it be a routine or an as needed medication, it is LAUSD policy that you must obtain a physician signed medication order, for which I have also attached the appropriate form to do so. Please note; Students are not permitted to carry medications on them or in their backpack without an LAUSD approved medication ordered that is signed by their physician. All medication orders should be brought to the main office and given to the School Nurse for review before the order can be implemented.
Thank you for your understanding and cooperation. Please know that we will not be able to accommodate for conditions/restrictions that we are not made aware of. These policies have been put in place to help us to ensure that your child is as safe as possible while at school.
